1/22/2024 EnchantaVaughn Approved QMHP-A inquired about Kaila’s thoughts, feelings, and emotions. QMHP-A recapped from the last session. QMHP-A reviewed with Kaila on expectations for session and addressing her anxieties. After discussion and review, QMHP-A explored calming techniques that would support her with calming skills such as (deep breathing, and relaxation). QMHP-A modeled effective ways Kaila could reduce negative thoughts and focusing on things that are good. Techniques that were taught to Kaila to assist her while modeling was educating her on how to become aware of the situation that is affecting her and replacing negative thoughts with a more positive and affirming thought. QMHP-A assisted Kaila by demonstrating a relaxation technique for Kaila to utilize daily when feeling overwhelmed, and stressed as she reported to wanting to yell and scream. After discussion and review, QMHP-A helped Kaila identify a list of her triggers and explored how she allows them to dictate her daily. QMHP-A taught Kaila how to apply learned coping skills from previous session to assist with her triggers and how to deal with them in a positive manner. Kaila appeared to be receptive to QMHP-A’s interventions and services that were provided to her during the session. Kaila reported that she engages in smoking marijuana at times when she feels as though she cannot handle certain things. Kaila reported just as she did when meeting with the psychiatrist, she smokes to overcome the stressors that she has at times in her life (i.e. her father and her daughter’s father). Kaila verbalized that she feels like just screaming and yelling sometimes as well as feeling that she wants to destroy things. Kaila verbalized that she struggles with using calming techniques as she becomes so frustrated and her negative thinking becomes problematic. Kaila was able to again write a list of her triggers and causes. She shared that she allows herself to become overwhelmed by things that she has no control.
